Community Connector in Swinton (part of the new Wellbeing Matters programme)

Inspiring Communities Together has employed a Community Connector as part of the Wellbeing Matters programme. Community Connectors are based in the heart of your community and will work with you to improve your health and wellbeing by connecting you to activities, services and organisations in your community.

A Community Connector will meet with you, listen to you and find out what matters to you most. You learn about your Community Connector through your GP, GP practice staff or Health Care Professional.
